Sure Fresh Foods, a chicken processing company, was fined $150,000 following the death of a welder in a water tank on 7 April 2006. The welder entered the 80,000 litre tank to repair a crack. When the welding torch was ignited the tank exploded. The source of combustion was found to be methane gas which had entered the tank from a water well. The company had not provided confined space training, procedures or gas monitoring equipment.
